Your First Visit

We want to thank you for choosing Center for Adult and Family Medicine for your health care needs. During your first visit, you will meet our staff, complete a few brief forms and, of course, meet your doctor. As your primary doctors, we will try to solve your current medical problem and detect or prevent other health problems. We hope to make the first visit not just an opportunity to deal with any medical concerns you have, but also a time to get acquainted with you.

Before your first visit with us, we will need to collect information on you and your current health. To help speed up this process, we have placed several of our forms online for your use. Please download these forms, fill them out prior to your visit, and bring them with you, along with your insurance card, to your first appointment. This will save you time in our office and make your appointment all that more efficient.

Note: If you arrive to your appointment without these forms you will be asked to fill them out at the time of your visit. As this may delay your appointment and cause inaccuracies (you may feel rushed), it is best to have the forms completed ahead of time.

Subsequent Visits

Each year, we need to update your file to ensure we have the correct personal, demographic, financial and privacy information. Please take the time to download the forms below, fill them out, and bring them with you on your next office visit if more than a year has passed.

Of course, you should notify us with any insurance and contact changes so that we can file your claims correctly.

We are required to collect all co-payments, non-covered services and deductibles at the time of service. These payments will be collected at the front desk.

We have payment options available, if necessary, so please ask to talk to our billing specialist.

What to Expect

As part of our commitment to your health, we recommend that every patient have a yearly exam to evaluate your overall health. Any chronic illnesses will need to be followed up every three months.

We hope that after your visit you will feel confident that you’ve made a wise decision in choosing our practice. If you are not comfortable with one doctor's practice style, you may reschedule your next appointment with another doctor in our practice.

Annual Preventive Physical Exam

Our MA/Nurse will escort you to the exam room, measure your height and weight, and take your blood pressure and temperature. Your physician will review the health questionnaire with you. You may be asked to undress and put on a gown in the privacy of the exam room. This enables the doctor to better evaluate your health. After the examination, your physician will suggest a treatment plan and future visits, if necessary.

Current Medical Issue

Our MA/nurse will bring you to the exam room and take your vital signs which include your height, weight, blood pressure and temperature. He/she will record the problem you are having into your chart. The doctor will then see you and treat the problem. Depending on the complexity of your condition, you may require follow-up visits. Your doctor will let you know if this is the case.

Chronic Medical Conditions

For our established patients, we will make an appointment for your lab work in advance and your exam at least 2 days later, so the lab results can be reviewed with you. For all lab results that are normal, we will mail you a card letting you know. If the lab results are abnormal, you will be called to come in and discuss the treatment plan with your doctor.

myDrforLife (online information access)

Center for Adult and Family Medicine uses electronic medical records to help provide your care. If you choose, you may communicate with our office securely over the internet by using myDrforLife. As a myDrforLife user, you will be given a secure password that will allow you confidential access to some of your private medical documents. such as:

lab reports
radiology reports
billing statements

Non-urgent appointments can also be made by accessing myDrforLife. Once you schedule your appointment online, we will contact you by phone to confirm the date and time.

Please note: You must make an office visit to discuss abnormal lab/xray results with your doctor or to address any questions you may have about your results.
